Ah yes, Johnathan. I wasn't thinking of it as his first time back. He appeared later on in the same season he died (season 5) but I guess technically, he was still a main at that time.

No. Two were mains (Clark and Chloe) and the third was a RG (2 episodes, Male, Alive, non powered, not in prison or coma, much older than Clark, never a main)

He was listed as the writer for all three episodes on IMDB. It it's not him, I got nuthin.
According to imdb "McCanlies pitched a series called Gotham which would have chronicled the rise of a young Bruce Wayne with Clark Kent as a recurring character. McCanlies' concept was turned into the series Smallville, and as of 2006 McCanlies continues to receive royalties for each episode produced."
