The obvious choice was Lex's project, the brother relationship of Lex and Julian, etc

Yet "twin" also referred to Clark and Bizzaro.

Im sorry but this is the best brainchild TWTB has came up with in a loooong time

There was a lot of speculation in the spoilers forum regarding the twin aspect to Gemini and that "Clark" was really Bizarro in this episode.

But yes, multiple levels of meaning can be found in just about every Smallville episode title if you give it a bit of thought.

Im sorry but this is the best brainchild TWTB has came up with in a loooong time I think there was actually more to it than what you've mentioned. I suppose you could say "the brother relationship of Lex and Julian", but I think it revolved more around:

Gemini = Cluster of Twins
1) Julian x 2 (Older version and Younger version)
2) Clark x 2 (BiZaRrO version and Real version)

Not sure, but to me - that's really more like a set of twins and not a cluster. So the episode title seems misleading somewhat. I was expecting more.
