I think Dr. Knox is supposed to be inspired by Vandal Savage, just as other weekly villains have been revelvant to various stories. The pilot's Jeremy Creek was based off Conduit, for example.

I was just researching Savage on Wiki and this was at the end:

"In Smallville, Dean Cain, who portrayed Superman in Lois & Clark: The New Adventures of Superman, guest stars as Dr. Curtis Knox, a character very similar to Vandal Savage, in the Season 7 episode Cure. In one scene Chloe says to Knox: "I came here for help, not to be cut open by Jack The Ripper wannabe." Knox replies: "I was Jack The Ripper"
