It's very simple really. If Lex had set up the appointment Lana would never know the pregnancy was faked. It had to be Lana needing medical care without Lex's knowledge.

He knows that she suspects he was involved in causing the bullet to end up where it did in Freak. He knows nothing about what she saw him do in Promise.
