When Clark said "he doesnt wanna tell her hes afraid of how she will react" he was obviously talking about himself and Lana aswell but doesn't he already know she will accept him?

Yes but he also knows she will be in danger knowing her secret and is not capable of keeping it a secret because she is too close to Lex.
