He was doing them at normal speed because he was concerned the paparazzi may be watching (as an ex-boyfriend of the soon to be Lana Luthor).

That was Lex to Chloe at the caves at the very end of Commencement, when she asks why they are there - shortly before she knocks him out and gets beamed to the arctic.

I don't know. You have the right context, I think, but the wrong boyfriend. There is a scene in X-Ray when Lana (actually Tina) goes to kiss Clark and he asks, "What about Whitney?" This is a similar situation.

1st instance of "I'm right here" was Ian Randall to Eric Summers in the utility room at Belle Reve in Asylum (he had merged with his other self) before Eric leeched Clark's powers again.
2nd instance was Lana to a frantic Clark at the farmhouse in Persona on his return from the FoS after Jor-El released him to tackle Bizarro. Lana was mortified shortly after to find that she'd been getting sweet with Biz, not Clark.
3rd instance was again Lana to Clark, this time in their bedroom in Requiem after both had got dressed at superspeed and Clark was waxing lyrical about never thinking he'd be able to work side by side with her.

That was young Lex to young Davis in Eternal, when asked if he was Warrior Angel.

The problem is, "Are you okay?" has been said more than 150 times (making it a definite contender in the "most repeated line" contest). Without more to go on, there's no way anyone is likely to come up with the single instance you are looking for. HU did give a correct answer.
